FORMS FOR OUR PARENTS

Below you will find forms that we need for each child.

Emergency Contact/Parental Consent Form - This is general information about the child, parents or guardians, emergency contact persons and the names of those people to whom we can release your child, your physician's name, address and phone number, and also information about any allergies or dietary needs of your child.

Child Health Report - Upon enrollment, each child must have a physical exam by a medical doctor and have up-to-date immunizations. He/she must have an annual exam thereafter. According to the Department of Public Welfare regulations, infants up to and including 18 months are required to have a physical every three months (3,6,9,12,15 and 18 months).

Child Questionnaire - This form gives our staff more detailed information about your child - likes, dislikes and fears. It helps us make your child more comfortable at the Center. There is a separate form for infants/pre-toddlers
